Registration


Your physician will give you an order for services to be performed at the hospital. We encourage you to pre-register at least two business days prior to receiving services. When calling, please have your order and insurance information ready to expedite the process.

If you are a patient at the hospital or at one of our other satellite locations, we will ask you to confirm the patient information we have on file from your last visit. Verifying this information ensures that our medical and financial records are accurate.

For outpatient services such as laboratory and x-ray, we encourage you to pre-register by telephone. You may contact a pre-service representative at 909.948.8166 weekdays, from 8 am to 5 pm.

Patients who have not pre-registered should report to the outpatient registration location 30 minutes before their scheduled service.


San Antonio offers convenient locations for most outpatient services:


Scheu Family Building

Located at 1100 San Bernardino Road, Second Floor, Upland, CA 91786:
Outpatient services include: Preoperative Center, laboratory, radiology (X-ray, ultrasound, CT scan, PET-CT scan), and the Women's Breast & Imaging Center. Open Monday through Friday from 7:00 am to 4:30 pm; closed Saturday, Sunday, and holidays.

Building 685

Located at 685 North 13th Ave., Upland, CA 91786:
Cardiac rehabilitation is open weekdays by appointment only. Closed Saturday, Sunday, and holidays.

Building 901

Located at 901 San Bernardino Road, Suite 300, Upland, CA 91786: 
Physical therapy is open weekdays by appointment only. Closed Saturday, Sunday, and holidays.

Rancho San Antonio Medical Plaza 

Located at 7777 Milliken Avenue, Rancho Cucamonga, CA 91730: 

Outpatient services include: laboratory, x-ray, mammography, and ultrasound. Open Monday through Friday from 7:00 am to 4:30 pm and on Saturday from 8:00 am to 12:00 pm.  Rehabilitation services (physical therapy) are available weekdays by appointment only. Closed Saturday, Sunday, and holidays.

Urgent care is open Monday through Friday from 9:00 am to 8:00 pm; Saturday, Sunday, and holidays from 9:00 am to 5:00 pm.

Sierra San Antonio Medical Plaza

Located at 16465 Sierra Lakes Parkway, Fontana, CA 92336: 
Urgent care is open Monday through Friday, 9:00 am to 5:00 pm; Saturday, Sunday, and holidays from 9:00 am to 5:00 pm. 

Eastvale San Antonio Medical Plaza

Located at 12442 Limonite Avenue, Eastvale, CA 91752: 

Urgent care is open Monday through Friday, 9:00 am to 5:00 pm; Saturday, Sunday, and holidays from 9:00 am to 5:00 pm. 



Hospital Main Campus

Located at 999 San Bernardino Road, Upland, CA 91786: 

A few outpatient services are exclusively provided at the main hospital; these include nuclear medicine, respiratory therapy, neurodiagnostic, and cardiology; all by appointment only.

Maternity Patients

We ask all maternity patients to pre-register within 60 days of their expected due date by contacting our admitting department at 909.920.4826, weekdays, 8:00 am to 4:00 pm.
