Daisy Award

The DAISY Award is an international recognition program honoring extraordinary Registered Nurses who demonstrate an unwavering commitment to provide compassionate patient care through exceptional clinical expertise.

Nomination Process:

If you would like to nominate an exceptional registered nurse for the DAISY Award, please submit your nomination digitally by clicking here to our online form. When you are finished, press "submit" and it will be emailed to the selection committee. If you would like to submit a paper form, click on the link below to download the PDF form. Please submit all nominations by the 15th of each month to be considered for the current month's award.

Adela is a compassionate and dedicated nurse whose positive attitude and willingness to help others make her an invaluable member of the team. While she has only been at San Antonio for one year, her commitment to teamwork, ever-present smile, and caring spirit have made a lasting impact on everyone. Adela was nominated by a colleague who recognized her exceptional clinical vigilance and commitment to patient safety. While monitoring a post-surgical patient, Adela identified subtle irregularities on the telemetry strip. She immediately reached out to the cardiac team to validate her findings. What initially appeared to be a minor concern was ultimately identified as a malfunctioning pacemaker. Adela represents the very best of nursing--she didn’t just “watch the monitor,” she interpreted the data with critical thinking and took decisive action to ensure the patient’s safety.
