Care4U Family Health Centers

Ensuring timely follow-up care after your hospital stay is an essential part of your entire care process. Most follow-up visits after discharge are just a checkup to see how you’re doing and to ensure there aren’t any complications. It’s also a great time to talk to a physician about your discharge plan, medications, and/or discuss any questions you may have.

The Care4U Family Health Center  is a San Antonio Regional Hospital outpatient service.

Care4U focuses on helping patients (who may not have access to a physician within one week of discharge) transition successfully to their home after their hospital stay. The goal is to reduce complications and avoid unnecessary hospital readmission. It does not replace the care of your regular physician.
