For Verifiers requesting verification of employment

We utilize The Work Number by Equifax to complete all requests for verification of employment and income for our employees. Please follow the following instructions.

To obtain proof of employment or income via The Work Number, you will need:

  • San Antonio Regional Hospital’s employer code, which is 19123
  • The employee's Social Security number
  • A permissible purpose for accessing the information as defined by the Fair Credit Reporting Act
  • Direct employee consent if income or salary information is needed
  • Credentialed access to use The Work Number
