Medical Records

RELEASE OF PATIENT INFORMATION
Medical records are confidential and are protected by state and federal laws. Although the medical record is the property of San Antonio Regional Hospital, the patient has the right to access information contained in their medical record. You may obtain a copy of the information in your medical record by signing an authorization to release the information. There is a fee for copies.

To obtain more information on how to receive a copy of your medical record, please contact Health Information Management weekdays, from 8 am to 4 pm, at 909.920.4750. To expedite your request, print out and complete the Authorization to Use and Disclose Health Information Form (link below). This request can be faxed (909.920.4745) or mailed to the Health Information Management Department for processing.


Birth Certificates

You may obtain a birth certificate by submitting an application at the San Bernardino County Recorder Hall of Records, 222 W. Hospitality Lane, San Bernardino, CA 92415-0022. The Hall of Records is open Monday – Friday, 8:00 am to 4:30 pm; closed weekends and holidays. Additional information can be obtained by calling toll-free 1.855.732.2575 or at sbcountyarc.org/services/vital-records/.

X-Rays

If you would like a copy of your X-rays, please contact our Health Information Management Department weekdays, from 8:00 am to 4:00 pm at 909.920.4750. X-rays are considered part of the patient's private medical record. To obtain a copy of your X-rays you must go to the Health Information Management Department in the main hospital.
